Operative Zugangswege zum lateralen Tibiaplateau

Abstract

The clinical outcome after treatment of tibial plateau fractures depends on the anatomical joint reduction. The main risk for tibial plateau fracture malreduction is insufficient intraoperative fracture visualization. Posterolateral fractures in particular are often inadequately visualized using standard approaches. However, in over 80% of complex intra-articular tibial fractures, this area is involved. In order to select the correct approach for the fracture extent, the classification of the fracture is important in addition to exact knowledge of the described approaches. In preoperative planning, the classification gives the possibility to choose the prone, lateral or supine position of the patient and the approach that allows the best possible visualization of the fracture. However, according to the principle of stepwise extension as needed, in addition to exact knowledge of the standard approaches, it is also important to know the extension possibilities and their risks.
