Interventioneller Mitralklappenersatz

Abstract

Patients suffering from significant mitral regurgitation (MR) frequently present with a complex comorbidity burden and therefore need sophisticated treatment algorithms. Besides established treatments of MR, such as surgical mitral valve repair and replacement or transcatheter edge-to-edge repair (TEER), transcatheter mitral valve implantation (TMVI) presented promising results over the last few years in patients not eligible for surgical or established interventional treatment. The TMVI has the potential to improve quality of care for those patients. To date, bioprostheses delivered by TMVI are mainly transapically implanted. Implementation of TMVI into daily clinical routine requires particular technical and administrative prerequisites to ensure adequate quality of care. Participation of cardiac surgery departments in establishment of the TMVI programs is of paramount importance to prospectively shape and perform these procedures. That particularly requires attendance of daily heart team meetings, implantation of transcatheter mitral valve prostheses, an active role in transseptal TMVI procedures, postprocedural care and scientific documentation of outcomes in these patients.
