In Germany, patients with severe diseases can be prescribed cannabis buds, extracts and synthetic cannabinoids whereby the costs can be covered by health insurance companies. The legislative authority did not restrict the prescription of cannabinoids to specific patient groups. Therefore, cannabinoids can be prescribed to every severely affected patient, independent from the underlying disease, if standard treatment has failed, resulted in unbearable side-effects or is contra-indicated due to other medical factors. According to regulations, cannabinoids can be prescribed as long as there is at least a slight chance of improvement of severe clinical symptoms. The challenge for the prescribing physician is to estimate a possible effect of medical cannabis regarding certain indications and diseases or clinical symptoms of the individual patient. This article provides an overview on possible treatment indications for medical cannabis in neurology and discusses and evaluates studies in movement disorders (Parkinson’s disease, atypical parkinsonian syndromes, dystonia, Huntington’s disease, tic disorders), multiple sclerosis, epileptic syndromes and motor neuron diseases in order to support the physician in the decision-making process of prescribing medical cannabis.
